It is rectangular in shape with a single nave with a continuous barrel vault roof, it has two bell towers at the ends; one of them is the smallest. From its beginnings it was also a health home to attend to poor natives and Spaniards, by royal order of Carlos V; Martín Guamán, father of Felipe Guamán Poma de Ayala, worked as a yanacona in its construction.
It is located on the street that bears the same name, Ciudad de Ayacucho.
